Özet
The most common causes of chronic kidney disease (CKD) are diabetes, hypertension, and vascular diseases. As the incidence of these diseases in society increases, the number of dialysis patients increases exponentially. The primary role of the family physician in kidney diseases is to refer the true patient who needs renal replacement therapy to a nephrologist on time. Dialysis is a form of renal replacement treatment (RRT) based on a fluid-solute exchange between the patient's blood and the dialysis solution through a semi-permeable membrane. After a while, patients with end-stage chronic renal failure need acute or chronic dialysis treatment included in the program with hemodialysis (HD), peritoneal dialysis (PD), or home dialysis. Family medicine health centers (FMHCs) are essential for the follow-up of chronic diseases. Family physicians can take part in therapy for comorbid conditions, such as atherosclerosis, hypertension and diabetes. Following up dialysis patients with a strategy including their family physician in the lead of a nephrologist is a cost-effective and timesaving way. A checklist, including cardiovascular risk management, diabetes control, anemia, vitamin deficiency, diet targets, exercise plan, psychological health counseling, laboratory follow-up, weight, and edema controls, is possible in their FMHC.
