The effect of the combination of tadalafil and dutasteride on lower urinary tract symptoms and erectile function in patients with and without a prostatic median lobe

Objective: This study aims to evaluate the effect of daily tadalafil plus dutasteride treatment on lower urinary tract symptoms (LUTS) and erectile function in men with and without a prostatic median lobe due to benign prostatic hyperplasia (BPH). Materials and methods: Between 2018 and 2022, 96 male patients aged ≥ 40 years with LUTS/BPH and erectile dysfunction (ED) were enrolled and divided into two groups based on the presence of a prostatic median lobe. Both groups received daily tadalafil (5 mg) and dutasteride (0.5 mg). IPSS, IIEF-EF, Qmax, prostate volume (PV), and post-void residual volume (PVR) were assessed before and after 24 weeks. PV and intravesical prostatic protrusion (length and volume) were measured using 3D transabdominal ultrasound after adequate bladder filling. Results: A total of 96 patients were included: 42 with a median lobe (Group 1) and 54 without (Group 2). No significant differences were observed between groups in baseline IPSS, IIEF-EF, PSA, or Qmax (p > 0.05). After 24 weeks of treatment, both groups showed significant improvements in IPSS, IIEF-EF, and Qmax. However, IPSS reduction was greater in Group 2 (p = 0.012), and post-treatment IIEF-EF (p = 0.034) and Qmax (p = 0.001) were also higher compared to Group 1. Conclusion: Daily tadalafil plus dutasteride is an effective and safe treatment for men with LUTS and ED. The presence of a prostatic median lobe may reduce treatment response. Further studies are warranted.
