Özet

Disease simulation and clinical deception may range from the unconscious processes (somatization) through the conscious illnesses (dermatitis artefacta etc.) to the falsification for deliberate gain (malingering). Among the self inflicted skin diseases, obsessive compulsive disorders (neurotic excoriations, trichotillomania etc), factitious diseases such as dermatitis artefacta and malingering must be differentiated. The difference between dermatitis artefacta and malingering lies in the intent of the patient. Malingering refers to feigning or significantly exaggerating symptoms for a conscious gain or purpose. In dermatitis artefacta the patient creates lesions or simulates a skin disease consciously but for a largely or wholly unconscious purpose. The clinical presentation of dermatitis artefacta is characteristic, and differs from that of other self inflicted skin diseases. Except where a special skin disease is mimicked, lesions do not conform to those of known dermatoses, are shrouded in mystery, appearing fully formed on accessible skin in bizarre or geometric forms. Most patients have a personality disorder; borderline features are common. Management is difficult, requiring a special form of doctor-patient relationship. Special forms of simulated diseases such as Munchausen's syndrome may also manifest with skin lesions. © 2010 by Türkiye Klinikleri.
