The effect of aprotinin to postoperative bleeding in aspirin using coronary artery patients ASPIRIN KULLANAN KORONER ARTER HASTALARINDA APROTININ'IN POSTOPERATIF KANAMAYA ETKISI

Özet
We have researched the effect of prophylactic use of aprotinin on postoperative bleeding and use of bank blood in 42 patients who had been operated on coronary artery bypass surgery in GATA Cardiovascular Surgery Department. 20 of these patients had been taken as control group (Group I) and according to the Royston protocol the remaining 22 patients had been given high dosage of aprotinin (Group II). There was no significant difference amongst the preoperative characteristics of the 2 groups. The amount of postoperative bleeding and the use of bank blood; 930 ml. and 1.7 unit in Group I and 325 ml. and 0.1 unit in Group II (p < 0.05). 2 of the patients had been reoperated because of bleeding in Group I. There was no mortality due to bleeding in both groups. At postoperative 6th hour in Group I and Group II; hemoglobin 9.1 gr/dl and 10.7 gr/dl (p < 0.05), thrombocytes 85.000/ml and 140.000/ml (p < 0.05), fibrin degradation products ≥ 40 μg/ml and ≥ 10 < 40 μg/ml (p < 0.05), bleeding time 10.8 sec. and 9.4 sec. (p < 0.05). No complication had been seen in Group II about aprotinin use. As a result; use of high dose aprotinin in preoperatively aspirin taking patients decreases the postoperative bleeding and use of bank blood.
