Özet
Objective: We detected eight cases with urinary tract infection due to Extended-spectrum beta-lactamase (ESBL) producing Klebsiella oxytoca in neonatal intensive care unit (NICU) of our hospital within a three-months period. We aimed to detect the source of an possible outbreak. Material and Method: Identification was confirmed by VITEK 2 bacterial identification system. Antibiotic susceptibility of the isolates were tested according to Clinical and Laboratory Standards Institute (CLSI) criteria. ESBL production was confirmed using cefotaxime and ceftazidime discs and their combination with clavulanic acid. Nucleic acids were extracted by boiling. Arbitrarily primed polymerase chain reaction (AP-PCR) was performed by using M13 primer (5'-GAG GGT GGC GGT TCT-3') as described before. Results: The outbreak involved eight patients. The conducted research showed, eight isolates from patients having the same phenotypic and genotypic patterns. The strains isolated from a healthcare worker and an incubator after environmental screening cultures, showed the same antibiotic susceptibility pattern as the patient isolates but had a distinct genotypic pattern. Conclusion: The evaluation of AP-PCR results indicated that the outbreak was caused by a single isolate, but results also showed different genotypic patterns apart from outbreak isolate for environmental isolates. Although we could not reveal the exact source of the outbreak, colonization of incubators, enteral feeding solutions and baby formulas by Klebsiella species is a known fact and also this kind of contamination findings are reported in other researches. After strict implemantation of control measures by hospital infection control comitee none of the urine cultures from NICU yielded the same K. oxytoca strain.
