Changes in thyroid function tests in patients undergoing cardiac surgery and patients in the intensive care unit Yoǧun bakim servisinde kardiyak cerrahi operasyonu geçiren hastalarda tiroid fonksiyon testlerindeki deǧişiklikler

Critical illness is associated with nonthyroidal illness syndrome (NTIS). NTIS is also observed after cardiac surgery. The study aimed to assess thyroid hormone changes after complicated and uncomplicated cardiac surgery and compare these with changes observed in the intensive care unit (ICU). Twenty post cardiac surgery patients, staying in intensive care unit (ICU) less than 48 hours (group 1), 20 post cardiac surgery patients, staying in the ICU more than 48 hours (group 2) and 20 patients followed at ICU for reasons other than surgery (group 3) were included. TSH, fT3, fT4, TT3 and TT4 were measured preoperatively and at postoperative 2 nd and 5 th days in groups 1 and 2 and at the first hours of ICU stay, followed by 2 nd and 5 th days in group 3. Although fT3 levels in group 1 remained unchanged, they decreased in group 2 and 3. Similarly TT3 levels were decreased significantly in group 2 and 3 and did not change in group 1. No change was observed in either fT4 and TT4 levels in all groups. TSH levels decreased only in group 2 and were stable in the other groups. Although thyroid hormone levels were stable in group 1, NTIS was observed in groups 2 and 3. Thus severity of the underlying condition seems to be important for the occurrence of NTIS after different clinical situations.
