Isolated Unilateral Internuclear Ophthalmoplegia After Trauma: A Case Report

Özet
Internuclear ophthalmoplegia (INO) is a clinical syndrome caused by a lesion in the medial longitudinal fasciculus (MLF), most commonly associated with multiple sclerosis or cerebrovascular diseases. Isolated unilateral INO due to trauma is exceptionally rare. This report presents the case of a 37-year-old woman who developed isolated unilateral INO following a blunt head trauma, caused by MLF infarction. The diagnosis was established via neurological examination and diffusion-weighted imaging. The patient’s symptoms were attributed to MLF infarction resulting from injury to perforating arteries or transient vasospasm. Her symptoms resolved completely within 48 hours without any specific treatment. This case highlights the need to consider traumatic isolated INO in the differential diagnosis and underscores the importance of meticulous neurological and radiological evaluation for accurate diagnosis.
