A comparison of the effect of high dose fentanyl-flunitrazepam and low dose fentanyl-propofol infusion on EEG spectral edge frequency and hemodynamics

Özet

The effects of high dose fentanyl- flunitrazepam and low dose fentanyl-propofol infusion anesthesia on the basis of electroencephalography (EGG) and hemodynamics were compared in the 18 patients undergoing coronary artery surgery. Parameters were recorded before induction as a control and during induction, intubation, incision, sternotomy, under hypothermia and after cardiopulmonary bypass (CPB). In the patients given high dose fentanyl-flunitrazepam (Group 1), heart rate (HR) decreased, however mean arterial pressure (MAP) increased during incision and sternotomy. Fentanyl administration caused significant decrease in SEF and increase in delta wave amplitudes and low frequency wave. Flunitrazepam given after fentanyl in Group 1 caused short term increase in SEF and propofol given after fentanyl in Group 2 caused no alteration in SEF. The hemodynamic changes in Group 1 and Group 2 gave out no significant correlation with the changes in SEF at the same intervals (p ~ 0.05). Despite of the lack of the correlation between EEG-SEF values and hemodynamics, EEG monitorization may be considered as a quality and safety enhancing method in patient monitorization for the patients undergoing cardiac surgery.
