Özet
Small cortical strokes are reported to cause selective impairment of finger and hand movements. We report a case of isolated weakness of the shoulder caused by cortical infarction. A 68-year-old, right-handed woman with vascular risk factors had acute pure motor paresis limited to her right shoulder muscles. Her cranial MRI showed a small infarction contralateral to the affected shoulder. This lesion was located at the central sulcus, more medially than the precentral knob. The precentral knob is reported to be the motor hand area, and has been proposed as a reliable landmark of the precentral gyrus. Our findings support this predicated topography.
