Özet

Objective: Pregnancy-related back pain is a common condition and it causes serious disability in activities of daily living of pregnant women. The aims of this study were to evaluate the source of back pain in pregnancy, to determine the risk factors and the effects of back pain on activities of daily living. Material and Methods: Fourty pregnant women with back pain, from 24 to 36 weeks gestation and aged between 16-39 years (mean 27,12±5,25 years), participated in this study. After taking detailed obstetrical history, all data obtained from ultrasonographic examination including gestational age, fetal gender, and plasental localization were noted. Localization and radiation of pain were evaluated using Quebec Task Force and pain intensity was detected using 0-10 cm Visuel Analog Scale (VAS). Lumbar lordosis, lumbosacral mobility, back pain during motion, posterior pelvic pain, pain radiated from symphysis pubis and sacroiliac joint were evaluated. Piedallu's sign was used to the detect the alignment of posterior superior iliac spine on horizontal plane. Existence of Laseque sign was noted. After careful examination, pregnant women with lumbar disc disease were excluded. The effects of back pain on activities of daily living were tested using Quebec Back Pain and Disability Scale. Results: Of the 40 pregnant women, 67,5% had posterior pelvic pain, 40% had lumbar pain, and 7,5% had both posterior pelvic pain and lumbar pain. The mean score of Quebec Back Pain and Disability Scale in pregnant women was 38.75±14.86 (min15,max70). The mean scores of Quebec Back Pain and Disability Scale in pregnant women with posterior pelvic pain, and lumbar pain were 38,62±15,51 and 41,00±14,12, respectively. Despite the lower mean quebec low back pain disability scores recorded in pregnant patients with posterior pelvic pain with regard to pregnant patients with low back pain, this difference was not statistically significant. Conclusion: Pregnancy related back pain may be arising from different anatomical sites. It is important to detect the source of back pain in pregnancy, because posterior pelvic pain and lumbar pain were different clinical entities considering both clinical signs and treatment methods.
