Congenital coronary anomalies are usually incidental, uncommon and asymptomatic. We present a case of a successful septal ablation of the first septal perforator artery arising from the circumflex artery in a patient with symptomatic hypertrophic cardiomyopathy. © The Author(s) 2012.
