Caregiver burden and its relationship with mood and insomnia severity in caregivers of patients with parkinson’s disease

Objective: This study aimed to investigate caregiver burden (CB) in caregivers of patients with Parkinson’s disease (PD) and to examine its relationship with insomnia severity, mood-related symptoms, and patients’ clinical characteristics. Methods: This cross-sectional study included 43 patients diagnosed with idiopathic PD and their primary informal caregivers. Disease stage was assessed using the Hoehn and Yahr (H&Y) staging system. CB was assessed using the Zarit Burden Interview (ZBI), insomnia severity using the Insomnia Severity Index (ISI), and anxiety and depressive symptoms in both patients and caregivers using the Hospital Anxiety and Depression Scale (HADS). Results: The mean age of caregivers was 56.58 ± 11.01 years, 60.5% were female, and the majority were spouses of the patients. Moderate and severe CB was identified in 34.9% and 32.4% of caregivers, respectively. Clinically significant anxiety symptoms were observed in 44.19% of caregivers, depressive symptoms in 65.12%, and sleep problems in 88.37%. CB was not significantly associated with patient age, disease duration, anxiety or depression scores, caregiving duration, insomnia severity, or mood-related symptoms. Caregivers’ HADS-A scores were negatively correlated with patients’ disease duration (r = − 0.347, p = 0.023), while caregivers’ HADS-D scores were positively correlated with caregivers’ body mass index (r = 0.362, p = 0.017) and patients’ HADS-D scores (r = 0.333, p = 0.029). Conclusion: Moderate to severe CB was present in more than two-thirds of caregivers of patients with PD. Psychological symptoms, sleep problems, and clinical characteristics of patients appeared to develop independently of CB. These findings underscore the multifactorial nature of CB and the need for individualized caregiver assessment during rehabilitation.
