Özet
Although adverse reactions after local anesthesia occur relatively frequently, true allergic reactions make up less than 1 % of the total adverse reactions to local anesthetics. If is often difficult to determine the mechanism of a prior adverse reaction from the history alone. The process of diagnosing an immunologic reaction to an antigen involves correlating a suggestive history with an appropriate immunologic test. Regarding immunologic tests for local anesthetic allergy, the actual hapten and a hapten-carrier complex have not been identified yet. Therefore, since the exact antigen may be different than the native drug, the value of immunologic tests that use the native drug is controversial. A safe local anesthetic can be identified for the individual patient through the use of an alternative drug, skin testing and incremental challenge. This article suggests a current approach to the evaluation of patients with a history of adverse local anesthetic reaction.
