The brainstem, a critical component of the central nervous system, serves as a crucial connection between the spinal cord and the higher brain regions. This chapter provides a comprehensive overview of the brainstem, focusing on its anatomy, functions, and clinical relevance. This chapter begins discussing the three main regions of the brainstem: the medulla, pons, and midbrain. It highlights key nuclei, pathways, and connections which contribute to important neurological processes. The role of the cranial nerves originating from the brainstem is also emphasized. Furthermore, this chapter highlights the most common neurosurgical approaches to the brainstem, and the reasons behind the choice of specific entry zones. Finally, it provides an introduction to some common pathologies affecting the brainstem.
