Elective major abdominal surgery for gastrointestinal cancer under clopidogrel (Plavix®) treatment: Case series of six patients

General surgeons faced a dilemma when patients on antiplatelet therapy need a major abdominal surgery. Data regarding the management of antiplatelet therapy in patients undergoing major abdominal surgical procedures were extremely limited and current guidelines were mainly based on a few retrospective cohort studies and case reports. The aim of this study is to represent the outcome of six patients who underwent major abdominal surgery for gastrointestinal cancer while under clopidogrel therapy. We retrospectively analysed the medical records of all patients, who underwent major abdominal surgery for gastrointestinal cancer in our hospital between January 2010 and August 2015. Patients under clopidogrel treatment at the time of surgery were identified. Patient demographic, pre-operative and post-operative data were collected. We performed six oncologic surgeries (1 pancreaticoduodenectomy, 1 total gastrectomy with D2 lymphadenectomy, 4 colon resection) for gastrointestinal cancer in patients who continued their clopidogrel treatment. Relaparotomy and massive transfusion was indicated for only one patient due to intraabdominal bleeding who underwent Whipple procedure. We can conclude that performing gastrointestinal cancer operations under clopidogrel therapy should result with significant bleeding complications according to our limited patients’ population data and perioperative management of these challenging cases should be indicated on an individual basis.
