Özet
Objective: Determination of maternal and fetal risks in patients who had vaginal birth after cesarean (VBAC) and evaluation of demographic and obstetric characteristics of those patients. Material and Methods: Sixty eight cases that had VBAC were retrospectively investigated. Obstetric histories of patients including age, gestational week, gravidity, parity, the numbers of abortions and previous cesarean, previous cesarean techniques and indications were recorded. Moreover, clinical features of current pregnancies at the time of presentation were recorded Descriptive statistics were used for statistical analysis of data. Results: The mean age for 68 cases included in the study was 25.8 and 3 cases had previous 2 cesareans. 65 cases had 1 previous cesarean. Twenty eight cases had cesarean in their first pregnancy and 40 cases had at least 1 vaginal delivery history prior to cesarean operation. All cases were referred to the clinic during 1st stage active phase of delivery and did not accept cesarean delivery. They were informed about risks and signed informed consent form. Uterine rupture (1.4%) was occurred in one case. That case was delivered via fetal vacuum and urgent laparotomy was performed following delivery, hemostasis was maintained with uterine rupture repair. Five newborn was admitted to the newborn intensive care unit. Among them, 3 had transient tachypnea of newborn and 2 had problems related to prematurity. However, all admitted newborns were discharged with cure. No fetomaternal mortality was seen. Conclusion: Vaginal birth can be performed under close observation for those with previous cesarean history, referred at active phase of delivery. In that case, "once a cesarean, always cesarean" rule can be disregarded and cesarean rates can be reduced. Copyright © 2013 by Tükiye Klinikleri.
