Özet

Purpose: To evaluate postoperative change in choroidal thickness (CT) in patients with anisometropic amblyopia undergoing keratorefractive surgery. Methods: Anisometropic amblyopic patients and nonamblyopic patients who underwent keratorefractive surgery were included in the study. The eyes were divided into three groups. Group 1 consisted of eyes with anisometropic amblyopia, group 2 were the nonamblyopic fellow eyes, and group 3 (control group) were nonamblyopic eyes which had undergone keratorefractive surgery. At the third postoperative month, the CT of these eyes were measured by Enhanced Depth Imaging OCT (EDI-OCT). The choroidal thickness (CT) was measured in the subfoveal area and at 500 micron intervals nasally and temporally. Results: Twenty-three anisometropic amblyopia patients with amblyopic and fellow eyes and 23 control eyes were enrolled. The mean subfoveal choroidal thickness (CT) was 387.3 ± 168.8 μm in group 1, 412.2 ± 88.8 μm in group 2 and 337.3 ± 99 μm in group 3 (P: 0.019). Group 1 and group 2 showed higher choroidal thickness (CT) in the nasal and temporal quadrants than group 3 (P: 0.03, P: 0.04). At the third postoperative month, central foveal choroidal thickness was 356.6 ± 115.5 μm in group 1, 375.1 ± 112.5 μm in group 2 and 284.4 ± 98.9 μm in group 3 (P: 0.071). Choroidal thickness (CT) in the nasal and temporal quadrants at the third postoperative month was also similar (P: 0.210, P: 0.103). Conclusions: The macular choroid is thicker in amblyopic eyes and non-amblyopic fellow eyes than in the nonamblyopic controls. Improved fixation after refractive surgery may normalize CT. Objectif: Évaluer les modifications postopératoires de l’épaisseur choroïdienne en OCT chez des patients atteints d'amblyopie anisométropique et ayant subi une chirurgie kératoréfractive. Méthodes: Les patients amblyopes anisométropes et les patients non amblyopes, qui ont subi une chirurgie kératoréfractive, ont été inclus dans l’étude. Les patients étaient divisés en trois groupes. Le groupe 1 comprenait des yeux présentant une amblyopie anisométropique, le groupe 2 des yeux non amblyopes et le groupe 3 (groupe témoin) des yeux opérés d'une chirurgie kératoréfractive sans aucune amblyopie. Au troisième mois postopératoire; les CT-scan de ces yeux ont été mesurés par imagerie en profondeur améliorée mode TCO (EDI-OCT). Le CT-scan a été mesuré au niveau sous-fovéolaire et à des intervalles de 500 microns en nasal et en temporal. Résultats: Vingt-trois patients avec amblyopie anisométropique, avec des yeux amblyopes et d'usage préférentiel et 23 yeux témoins ont été recrutés. Le CT-scan sous-fovéolaire moyen était de 387,3 ± 168,8 μm dans le groupe 1, de 412,2 ± 88,8 μm dans le groupe 2 et 337,3 ± 99 μm dans le groupe 3 (p : 0,019). Les groupes 1 et 2 avaient un CT-scan plus épais dans les quadrants temporaux et nasaux que le groupe 3 (p : 0,03, p : 0,04). Au troisième mois postopératoire, l’épaisseur fovéale centrale de la choroïde était de 356,6 ± 115,5 μm dans le groupe 1, 375,1 ± 112,5 μm dans le groupe 2 et 284,4 ± 98,9 μm dans le groupe 3 (p : 0,071). Le CT-scan au troisième mois postopératoire dans les quadrants nasaux et temporaux était également similaire (p : 0,210, p : 0,103). Conclusions: Le CT-scan maculaire est plus épais dans les yeux amblyopes et les yeux dominants non amblyopes que les témoins non amblyopes. Une fixation accrue après une chirurgie réfractive peut amener le CT-scan à se transformer en valeurs normales.
