Introduction: Priapism, an uncommon urological emergency, is a pathologically prolonged and painful penile erection, usually unassociated with sexual desire or intercourse. Causes include generally certain medications, although the mechanism for drug-induced priapism is unknown. Sometimes, a cause related to priapism is not found. We report our experience on patients with priapism, applying to our clinic, during 1999-2002. Patients and Method: 16 patients with priapism applied to our clinic during January 1999 and April 2002. The mean age of patients was 35.5 (18-67). The mean lasting time of erection was 10.5 h (4-48). The causes of the patients were included 11 intracorporeal injection of papaverine (after penile doppler ultrasonography or auto-injection therapy of erectile dysfunction), 3 unknown causes and 2 drug-induced priapism (1 testosterone and 1 chlorpromazine). Results: 13 cases were treated with medical treatment or intracavernosal aspiration and/or irrigations easily. But, intracavernosal aspiration and irrigations failed in 3 cases. All of these 3 cases were treated surgically with a shunt. Cavernous spongiosal shunting failed in 2 cases whose erection lasting over 24 h. We performed penile prosthesis in these cases. Conclusions: The etiologies of priapism are numerous and diverse. The applying an erection time is important to succeed in management. Generally, the conservative managements can be successful. But, the conservative managements will be unsuccessful at some of patients applying after a long period.
