Matriderm and platelet-rich plasma combination in the treatment of recurrent carpal tunnel syndrome: a new approach

Background: The increased recurrence of clinical and electrophysiological findings and absences of their regressions were reported in some cases after revision surgery in recurrent carpal tunnel syndrome (CTS). In this study, we aimed to see the effect of Matriderm and platelet-rich plasma (PRP) on the clinical and electrophysiological findings in patients with recurrent CTS. Methods: The study included 36 patients with recurrent CTS. They were randomly divided into four groups. In group 1, classical revision surgery was applied; in group 2, Matriderm was applied; in group 3, PRP was applied; and in group 4, PRP and Matriderm together were applied. Patients were administered with Quick Disability of the Arm, Shoulder, Hand (Q-DASH) and Visual Analogue Scale (VAS) and underwent electrophysiological tests (sensory nerve action potential, sensory velocity, sensory latency, compound motor action potential, motor velocity, and motor latency) in the postoperative first year. Preoperative (gender, age, classification, duration of recurrence symptoms) and postoperative (recurrence after revision surgery and follow-up time) descriptive characteristics of the patients in both groups were collected. Results: CMAP, Motor velocity and motor latency values in group 4 were significantly different from other groups. Other parameters in group 3 and group 4 were significantly different from the values in group 1 and group 2, but there was no statistically significant difference between group 3 and group 4 in terms of these values. Conclusions: As a result, it can be concluded that the use of Matriderm and PRP together in the revision CTS surgery reduces the recurrence of symptoms, thus improving the recovery after the surgery.
