Özet

The aim of this study was to evaluate of the left ventricular (LV) myocardial radial and circumferential functions in the patients with rheumatic mitral stenosis (MS) by the strain (S) and strain rate (Sr) echocardiography. The study included 30 isolated rheumatic MS patients and 30 healthy subjects. Conventional and color tissue Doppler echocardiographic recordings were obtained from standard apical and parasternal axis imaging. Radial S and Sr measured from anterior wall, circumferential S and Sr measured from septal and lateral walls at the LV short axis imaging. Additionally time to peak systolic S (Ts) and Sr (Tsr), tissue Doppler systolic (Vs), early (Ved) and late (Vad) diastolic velocities were analysed. There were no difference between patients and controls for LV dimensions and systolic functions. Mean mitral valve area measured 1.4±0.3 cm2 in the patients. S, Sr, Vs and Ved values in the patients significantly lower than in the control subjects. There were no differences between patients and controls for Ts and Tsr, and Vad. Tsr were significantly lower than Ts in the all subjects. Left ventricular myocardial radial and circumferential functions are impaired in the patients with rheumatic MS who had normal systolic function in conventional echocardiography. However Ts and Tsr values are similar in the patients and healty subjects. Radial and/or circumferential deformation indices (S, Sr) acquired from LV short axis imaging may be used as practice and alternative method for the detection of early subclinic LV dysfunction in the rheumatic MS patients.
