OBJECTIVE: To show the correlation of anatomical and functional recovery of fertility and menstrual cycles following hysteroscopic surgery by using second-look hysteroscopy. STUDY DESIGN: Sixty-seven cases underwent operative hysteroscopy due to intrauterine adhesions, uterine septum, endometriyal polyp and submucous fibroids enrolled in this study. All patients have undergone second-look hysteroscopy, following a withdrawal bleeding after hormonal therapy. Intrauterine adhesions diagnosed by second-look hysteroscopy were operated by repeat operative hysteroscopy. Laparoscopy was performed when needed especially in cases with severe intrauterine adhesions. Repetitive diagnostic and operative hysteroscopies were performed with persistent intrauterine adhesions, until sufficient endometriyal cavity was achieved. RESULTS(s): Nine of the twenty-eight patients who have undergone operative hysteroscopy due to septum resection had adhesions diagnosed by second-look hysteroscopy. Secondary adhesions reoccurred in 14 out of 19 cases who have undergone hysteroscopy for adhesiolysis and one in nine patients operated for submucous fibroids. Although none of the patients treated for endometriyal polyps formed adhesions, a total number of twenty-four patients were shown to have adhesions following operative hysteroscopy (p<0.05). Eight of the 12 patients who have undergone adhesiolysis by third-look hysteroscopy showed adhesions in the fourth-look hysteroscopy (66.7%). Two of the cases were classified to have severe adhesions (16.6%) while one had moderate (8.3%) and one had mild adhesions (8.3%). These, also have undergone adhesiolysis. CONCLUSION(s): We believe that second-look hysteroscopy either diagnostically or therapeutically is a reasonable approach following operative hysteroscopy for any reason, especially in cases with Asherman syndrome and intrauterine septum.
