Gastrocolic fistula associated with gastric lymphoma

Introduction: Gastrocolic fistula is a rare presentation of malignant diseases of the gastrointestinal tract. Malignant gastrocolic fistula is most commonly associated with adenocarcinoma of the transverse colon and gastric cancer but gastric lymphoma is a rare but important cause of gastrocolic fistulas. We report a case of a 45-year-old male with non-Hodgkin lymphoma complicated with gastrocolic fistula. Case report: A 45-year-old mentally retarded male presented with melena and recurrent vomiting. Our patient presented with the characteristic triad of symptoms associated with gastrocolic fistula: diarrhoea, weight loss and faeculent vomiting. Medical history and physical, radiological and endoscopic examination were important for diagnosis. On computed tomography asymmetric gastric wall thickening was detected. On upper gastrointestinal endoscopy an ulcer was seen in the antrum with a fistula opening to the transverse colon. Multiple biopsies were taken. It was reported as diffuse B type non-Hodgkin lymphoma. Radical en-bloc surgeries including transversecolectomy and subtotal gastrectomy and then adjuvant chemotherapy were performed. Conclusions: Gastrocolic fistula should be suspected in patients with chronic diarrhoea and vomiting of unknown cause with a high suspicion of gastrointestinal malignancy. Copyright © 2010 Cornetis.
