Özet
Diaschisis has been simply described as the cerebral counterpart of spinal shock. Because the brain circuitry is relatively more complex than that of spinal cord, understanding the mechanism underlying cerebral diaschisis is more difficult. Sudden, brief and frequent attacks of dysarthria and ataxia were rarely reported excluding demyelinating disorders. This case report documents paroxysmal dysarthria and ataxia (PDA) together with midbrain infarction. We aimed to explain the mechanisms of parietal hypoperfusion due to diaschisis after midbrain infarction and PDA. Conclusion: We hypothesize that the parietal hypoperfusion which occurs because of cerebellothalamocortical pathways dysfunction after midbrain infarction might be the cause, PDA might be the outcome and the symptomatology occurs due to parietal diaschisis.
