Background/aims: This retrospective study was carried out to observe the efficacy and safety of papillotomy with a needle-knife papillotomy when biliary access could not be achieved by the standard method in cases undergoing endoscopic retrograde cholangiopancreatography (ERCP). Methods: The needle-knife papillotomy was used on 52 patients. Inability of an experienced endoscopist to achieve opacification or cannulation of the common bile duct by the standard method within 15 minutes was accepted as an indication for use of instrument. Results: In the first session, bile duct cannulation was achieved in 39 out of 52 patients (75%). Three to seven days following the first procedure, it was established in nine of the other 13 patients (17%). Thus biliary access was successful in 48 cases overall (92%). Percutaneous procedure was performed on two of the remaining cases and surgery on the other two. Perforation was the only complication, which occurred in three patients (5.7%), but only one of them required surgical treatment. Conclusion: These results suggest that in selected cases, papillotomy with the needle-knife papillotome is a safe and quite efficient method of treatment which should be considered before a percutaneous procedure and surgical operation.
