Özet

Conventional ascitic fluid analysis is widely insufficient for even a crude differential diagnosis of ascitic fluid. Recently, there is an effort to optimize ascitic fluid analysis. The aim of this study was to determine the significance of some new parameters in ascitic fluid analysis. The study was carried out on 50 patients. Subjects were divided into two groups: malignant ascites (n: 25), and non-malignant ascites (n: 25). The diagnostic accuracy of common biochemical parameters in ascitic fluid (ascitic fluid total protein, ascites to serum ratio of total protein, ascitic fluid lactic dehydrogenase, ascites to serum ratio of lactic dehydrogenase) were compared with those of carcinoembryonic antigen, serum-ascites albumin gradient, ascitic fluid cholesterol and ascitic fluid fibronectin. There were significant differences between two groups in all parameters studied (p<0.001). In respect of the diagnostic accuracy of ascitic fluid total protein was 74%, that of ascites to serum ratio of total protein 86%, that of ascitic fluid lactic dehydrogenase 80%, that of ascitic fluid carcinoembryonic antigen 80%, that of serum-ascites albumin. gradient 84%, that of ascites fluid cholesterol 70% and that of ascites fluid fibronectin 94%. Consequently, although the ascitic fluid cholesterol, ascitic fluid carcinoembryonic antigens and serum-ascites albumin gradient had no dominance to the common parameters, the diagnostic accuracy of ascitic fluid fibronectin appears to be considerably superior to common parameters.
