One of the most frequent complications of hepatic hydatid cysts are communication of cysts with the bile ducts. Scolocidal agents may also pass to bile ducts during surgery and may cause undesired effects. This study was designed to evaluate the effects of the most popular scolocidal agents on the hepatopancreaticobiliary system. In this experimental study, the effects of 5% Polyvinyl prolidone (PVP) iodine, 0.5% silver nitrate, 20% sodium chloride and 95% ethyl alcohol on the hepatopancreaticobiliary system were studied. Fifty guinea pigs weighing 550-950 gr were used and 2 ml of scolocidal agent was injected into the gall bladder. Blood values of ALT, AST, alkaline phosphatase, gama glutamyl transferase and amylase were studied preoperatively and 48 hours postoperatively. Plasma Na, K and Cl were studied for hypertonic saline, silver nitrate and ethylene alcohol groups. The liver, pancreas, gall bladder and common bile duct were resected for histopathological examination. The abnormalities of liver function tests were found to be significant in all groups including control groups. An elevated amylase level was observed only in the 95% ethyl alcohol group. Blood values of Na and Cl were found to be significantly elevated in the 20% Na Cl group but decreased in the 0.5% AgNO3 group. Ethyl alcohol was found to be the most toxic agent to liver, gall bladder and common bile duct and severe pathologic changes were observed in pancreas. 20% NaCl was the second most toxic agent to these organs while 0.5% AgNO3 was less toxic to liver and common bile duct and 5% PVP-iodine had the least toxic effect on pancreas and gall bladder. All scolocidal solutions used in this study had toxic effects on the hepatopancreaticobiliary system, with ethyl alcohol having the most toxic effect. They should not be injected intracystically if there is any suspicion of cyst communication with the biliary tract.
