Özet

Nasal septal perforation is defined as an anatomical defect in any portion of the cartilaginous and mucosal areas of the septum. Trauma to the mucosal membrane covering the septal cartilage substantially decreases the blood supply of the septum and the contraction that occurs in the period of recovery of the perforation may expand the defect. Many causes such as previous traumas, septal surgeries, inflammatory diseases, and use of various nasal sprays may lead to the formation of nasal septal perforation (NSP). Even though the majority of individuals having NSP are asymptomatic, a substantial minority of them experience devastating symptoms including crusting, epistaxis, postnasal drip, and nasal blockage. Unfortunately, the most common cause of perforation is the iatrogenic injury, frequently as a result of previous nasal septal surgery. Conservative treatments such as nasal irrigation, application of topical antibiotics, or prosthesis like septal buttons may help in reducing bothersome symptoms; however advanced operations should be scheduled for affected patients who do not respond well to the conservative remedies. Several operative techniques like interpositional grafts and local flaps had been described in the literature. Nevertheless, operative procedures are not well standardized and the important factors for efficient surgery have not been fully enlightened.
