Özet

Idiopathic or spontaneous perforation of the common bile duct in neonates and infants is rarely seen. A male infant whom was previously hospitalized at neonatal unit for investigation of convulsion etiology and that had two sepsis attacks was developed cholestasis. It was found in ultrasonography generalized ascites in abdomen and nuclide stasis on choledoc region in scintigraphy. He had complaints of abdominal distention and jaundice; at surgery biliary ascites and a perforation at the common bile duct were found. Primary repair of the perforation site and drainage was done. Postoperative period was uneventful. We couldn't find any cause of common bile duct perforation in our patient and recurrent sepsis episodes was considered as possible etiology. Although, simple drainage is enough, primary repair of perforation site can provide faster response to treatment. Common bile duct perforation can occur as acute and chronic form. Especially in chronic form diagnosis can be difficult. Spontaneous biliary tract perforation must be keep in mind in infant with jaundice and diffuse ascites, even there is no peritonitis sign.
