The diagnostic value of CEA in biliary tract carcinomas which can not be diagnosed by conventional methods remains controversial. The aim of this study was to investigate whether serum and bile CEA levels contribute to differentiation of benign and malignant disease in the biliary tract. Twenty one patients with cholangiocarcinoma, 17 patients with bening stricture and 27 patients with cholelithiasis were included in this study. Serum CEA levels were determined by radioimmunoassay, whereas bile CEA results were obtained by enzyme immunoassay method. Serum CEA levels were 92.5±65,8, 1.3±0.6 and 1.6±1 in cancer benign stricture and cholelithiasis patients respectively. Serum CEA levels in cancer patients were significantly higher when compared to those of the two other groups. There was no significant difference between the benign stricture and cholelithiasis patients with respect to serum CEA levels. Bile CEA levels were 9.2±5, 1.3±0.5 and 0.8±0.5 in cancer, benign stricture and cholelithiasis patients respectively. Cancer patients had significantly higher bile CEA levels than those of the benign stricture and cholelithiasis groups. The benign group also had significantly bile CEA levels than those of cholelithiasis patients. A significant correlation was found between serum and bile CEA levels in cancer patients. In conclusion, serum and bile CEA levels are high and show a positive relationship with each other in patients with cholangio carcinoma. Both bile and serum levels have equal diagnostic value, and may be used in differentiating benign and malignant biliary tract disease.
