Özet

Burns are still one of the most serious traumas faced by adults and children all over the world, especially in underdeveloped countries. The first 24 hours following the burn trauma constitute the most important part of the treatment. The most important points in burn treatment are first aid interventions to be made to the patient at the time of the burns and communicating with the burn unit/center by sharing burn photos. The cooling process under running water in the first 10-30 minutes prevents the burn from going deeper and reduces the pain. Starting the necessary fluid therapy by calculating the burn percentage according to the photos shared before the patient’s transport prevents fluid loss in the burn. If there is additional trauma accompanying the first evaluation in the hospital, it is necessary to first intervene with the principle of “forget the burn.” The treatment of the patient, who is stabilized outside the burn, is planned considering the cause of the burn event, the degree of burn, and the areas exposed to the burn in the burn unit/center. While conservative daily dressings are sufficient in superficial burns, deep burns are treated either conservatively or with early escharotomy, escharectomy, fasciotomy and graft application. Morbidity and mortality increase in the case of concomitant trauma, inhalation burn, existing cardiac or lung disease, immunodeficiency, and a more than 30% area of the burn. Correct and timely first aid, appropriate fluid replacement during transport, and rapid referral to the right center are the most important points in burn treatment.
